Psychotherapeutic Approaches: Psychoanalytic, Behavioral, CBT, REBT & Humanistic

16 min read·CUET PG Psychology

Psychotherapy represents the informed and intentional application of clinical methods and interpersonal stances derived from established psychological principles to assist individuals in modifying behaviors, cognitions, emotions, and personal characteristics. In the CUET PG Psychology examination (HUQP20), psychotherapeutic interventions form a core, high-weightage domain.

Key examination themes emphasize:

  1. Classical Psychoanalysis: Transference, countertransference, resistance, dream work, and defense analysis.
  2. Behavior Therapy & Modification: Joseph Wolpe's Systematic Desensitization and Reciprocal Inhibition; Flooding vs. Implosion; operant contingency management and Token Economies.
  3. Cognitive & Cognitive-Behavioral Therapies: Aaron Beck's Cognitive Therapy (CT) and collaborative empiricism; Albert Ellis's Rational Emotive Behavior Therapy (REBT) and the ABCDEF framework.
  4. Humanistic-Existential Modalities: Carl Rogers' Person-Centered Therapy and the three necessary and sufficient conditions; Viktor Frankl's Logotherapy; Fritz Perls' Gestalt Therapy; Eric Berne's Transactional Analysis (TA).

1. Classical Psychoanalysis and Psychodynamic Psychotherapy

Originating with Sigmund Freud, classical psychoanalysis posits that psychological symptoms are symbolic compromises reflecting unresolved, repressed unconscious conflicts between primitive instinctual drives (Id) and moral internalized prohibitions (Superego), mediated by the Ego.

DimensionCore PrincipleClinical Implementation
Ultimate Goal"Wo Es war, soll Ich werden" ("Where Id was, there shall Ego be")Transforming repressed unconscious impulses into conscious awareness to achieve intellectual and emotional insight.
The Fundamental RuleFree AssociationAnalysand verbalizes every passing thought, memory, and feeling without censorship, moral editing, or conversational restraint.
Therapist StanceAnalytic Neutrality & AbstinenceTherapist serves as a neutral "blank screen" (avoiding self-disclosure or gratification of demands) to facilitate clear transference projections.

Core Psychoanalytic Techniques

  1. Free Association:
    • The cornerstone technique where the analysand lies on a couch facing away from the analyst, reporting whatever emerges into consciousness without self-editing.
    • Epistemology: Slips of the tongue (parapraxes or Freudian slips), sudden blocks, and associations reveal associative pathways leading to repressed pathogenic complexes.
  2. Dream Analysis ("The Royal Road to the Unconscious"):
    • Freud distinguished between:
      • Manifest Content: The conscious, remembered narrative of the dream.
      • Latent Content: The true, repressed unconscious wish or instinctual impulse.
    • Dream Work: The cognitive transformations executed by the unconscious to disguise the latent dream thoughts into the manifest narrative:
      • Condensation: Multiple unconscious ideas or figures combined into a single composite image.
      • Displacement: Emotional energy shifted from a threatening central idea to an innocuous peripheral symbol.
      • Symbolism: Neutral objects standing in for primal instinctual themes (e.g., elongated objects representing the phallus).
      • Secondary Revision: The waking ego reorganizing the dream into a coherent, logical storyline.
  3. Analysis of Transference:
    • Transference: The unconscious displacement of feelings, drives, expectations, and attitudes experienced toward significant childhood figures (parents) onto the analyst.
    • Positive Transference: Attachment, idealization, or affection toward the analyst.
    • Negative Transference: Hostility, resentment, and suspicion directed at the analyst.
    • Transference Neurosis: The critical phase of analysis where the patient reenacts their core infantile neurosis within the therapeutic relationship. Resolving this relationship provides therapeutic cure.
    • Countertransference: The analyst's unconscious emotional reactions to the patient's transference. In classical analysis, countertransference was viewed as an obstacle to be minimized through the analyst's personal training analysis; modern psychodynamic therapy views it as a valuable diagnostic instrument revealing the patient's interpersonal impact.
  4. Analysis of Resistance:
    • Any client action, thought, or behavioral pattern that obstructs the analytical process to prevent unconscious anxiety-provoking material from breaking into conscious awareness (e.g., arriving late, missing sessions, sudden intellectualizing, sudden silence).
  5. Interpretation and Working Through:
    • Interpretation: The analyst points out the hidden meaning, defenses, and unconscious motives underlying the client's associations, dreams, or transference.
    • Working Through: The repetitive, elaborate exploration of interpretations across diverse life contexts, allowing intellectual insight (knowing what caused it) to mature into emotional insight and behavioral change.

2. Behavior Therapy and Modification

Behavior therapy rejects internal unobservable constructs (id, complexes) and applies principles of classical conditioning, operant conditioning, and observational learning to eliminate maladaptive behaviors and establish adaptive responses.

1. Joseph Wolpe's Systematic Desensitization

  • Theoretical Basis: Reciprocal Inhibition: "If a response antagonistic to anxiety can be made to occur in the presence of anxiety-provoking stimuli,\text{"If a response antagonistic to anxiety can be made to occur in the presence of anxiety-provoking stimuli,} so that it is accompanied by a complete or partial suppression of the anxiety responses, the bond will be weakened."\text{so that it is accompanied by a complete or partial suppression of the anxiety responses, the bond will be weakened."}
  • Deep physiological relaxation suppresses sympathetic autonomic arousal; since autonomic arousal (anxiety) and somatic relaxation are neurologically incompatible, relaxation inhibits anxiety.
  • The Three Standard Procedural Phases:
    1. Relaxation Training: Patient masters Progressive Muscle Relaxation (PMR), developed by Edmund Jacobson, learning to systematically tense and release 16 major muscle groups.
    2. Construction of the Anxiety Hierarchy: Developing an ordered list of 10 to 20 stimulus situations evoking graduated anxiety, calibrated using the Subjective Units of Distress Scale (SUDS), ranging from 00 (complete calm) to 100100 (worst imaginable panic).
    3. Desensitization Pairing: In imagination (imaginal desensitization) or reality (in vivo), the client visualizes the lowest hierarchy scene while deeply relaxed. If anxiety surfaces, the client signals, halts visualization, and reinstates relaxation. Once a scene can be visualized with zero anxiety, the therapist advances to the next step.
